Hybrid model of care for cardiovascular diseases Compared to routine hospitalisation , [email protected] ([email protected]), as hybrid model of care for patients with chronic diseases, could improve outcomes and reduces costs. Older Adults with Heart failure (HF) are often hospitalization due to exacerbations. Although such hybrid models care, like the Structured telephone support (STS) or telemonitoring for adult patients with HF, have been subject to clinical guidelines, a number of key factors need to be considered for moving telemedicine from pilots to scale. |

Digital meets home hospitalization care: a patient testimonial In a rapidly evolving digital world, the challenge lies in defining how digital technology could be most beneficial to the patient. As chronic conditions, such as heart failure, are dominantly present in the elderly generation resulting in higher in-hospital complication rates, there is a need for home care that is comfortable, reliable and highly qualitative. Enabling home hospitalization for acutely decompensated heart failure patients, with the support of an integrated digital platform and the care of a specialized nurses, could pave the path for hybrid care at home. This hybrid integrated care model is being explored in the Interreg NWE-Chance project supported by both technological, academic and clinical partners and will be highlighted in this presentation. |

From bricks to bytes and beyond: organisational implications of home hospitalisations Although there is a vast interest in home hospitalisations, this hybrid care model is still in its infancy. One of the main reasons is experienced boundaries when it comes to implementing a home hospitalisation strategy. We will dive into this and address the different organisational implications of home hospitalisations with among others clinical, technical, human resources and economic aspects. Furthermore, we will share lessons learned so far by NWE-Chance.

Background: the NWE-Chance project
Although there is a vast interest in home hospitalisations, this hybrid care model is still in its infancy. NWE-Chance is an Interreg North-West Europe funded project conducted by a consortium of ten partners in three countries (The Netherlands, Belgium, United Kingdom).
Within NWE-Chance, promising integrated eHealth applications will be developed for admitting heart failure patients at home and the feasibility of both technology and supporting care process will be assessed by three hospitals.
NWE-Chance will end with the launch of an innovation hub to bring stakeholders together and to share knowledge with respect to home hospitalisations. This presentation will address the different organisational implications of home hospitalisations (with among others clinical, technical, human resources and economic aspects) and will share the lessons learned so far.
